Internship: 

To complete the insurance procedures, the FR-059 Mandatory Internship Application Form, stamped and signed, must be submitted to the department's internship committee no later than 7 days before the start date of your internship.

Procedures to Follow After the Internship

1. Fill out the internship logbook available on the website by answering all the questions at the end.
2. Ensure that the FR-060 Internship Evaluation Form is completed by the workplace where the internship was conducted and submit it in a sealed envelope along with the internship report.
